Care Coordination

HIV care coordination is specialized case management for persons living with HIV/AIDS. Each client is assigned a care coordinator upon intake and will continue working with that individual to access health coverage, medical care, SNAP benefits, TANF, SSDI/SSI, mental health referrals, access to outside community services, and social support. Clients enrolled in Care Coordination are eligible for other client services, including the following financial assistance programs:

Direct Emergency Financial Assistance: Helps supplement client income by providing financial assistance for housing, utilities, prescription assistance, and transportation.

Benevolent Fund: Provides financial assistance to supplement costs associated with prescription drug co-pays and emergency services. Client eligibility is determined on a case basis.
