Care Coordination
HIV Care Coordination is a program funded by the Indiana State Department of Health. The mission statement of the Care Coordination Program is to assist those living with HIV disease with the coordination of a wide variety of health and social services. It respects cultural diversity, emphasizes confidentiality, and strives to ensure the client's freedom of choice and self-determination. Its comprehensive and compassionate services are rendered in a safe, secure and non-judgmental environment and are provided at no cost to the client.
ARG's Care Coordinators provide the following services to our nearly 200 HIV+ clients in 11 southwestern Indiana Counties:
- Comprehensive bio-psycho-social assessment
- Referrals to mental health counselors, including the agency's Supportive Care Substance Abuse Specialist
- Referrals to medical providers/infectious disease specialists
- Referrals for financial assistance
- Assistance completing medical insurance applications (including private pay, Indiana Comprehensive, Healthy Indiana Plan, Medicaid and others)
- Supportive listening
- Assistance applying for housing (Section 8, HOPWA, project-based, etc.)
